Maintenance-focused Savings vs per-visit Complete Chiropractic Treatments

Monthly Package

$70 per month

Includes: 4 chiropractic visits with treatment during the month purchased (up to one visit per week).

Does not include the initial visit unless purchased prior to the initial visit.
Valid for four (4) visits within the four (4) weeks following purchase.
Package visits must be used within the purchase period; unused visits expire.

5-Visit Package

$100 payment

Includes: 5 chiropractic visits with treatment.

$100 for 5 visits after the initial visit, unless purchased prior to the initial visit.
Package visits are applied to eligible visits as used; unused visits expire per office policy.
Self-pay wellness package for maintenance and convenience.

Summary of Terms and Benefits:
• This SpineAlign Chiropractic Wellness Membership Program is for the sole purpose of detection and correction of vertebral subluxation and is NOT for the diagnosis or treatment of any symptoms, diseases or conditions.
• All memberships are based on monthly auto-debit or annual prepayment by credit card, cash or check.
• All memberships are non-transferable.
• Fees charged are a retainer fee which allows access to the office during normal business hours.
• Members will receive a 100% discount on chiropractic adjustments delivered during normal business hours, one per week. Additional treatments are charged at our current treatment rate.
• This agreement is not insurance. SpineAlign Chiropractic assumes no risk, monetary or otherwise.
• No cash or credit refunds will be given for unused services. It is the Practice Member's sole responsibility to receive care per the chiropractor’s recommendations.
• We reserve the right to cancel membership at any time for any reason.
• Membership Fee Guarantee: Price protection guarantee – price remains the same for a minimum of 12 months as long as you are a member.
